At Kannact, we're helping patients with chronic diseases manage their health with active monitoring and personal coaching. The program is paid for by employers and on day one participants receive connected devices (to measure blood pressure, sugar, oxygen, etc.), a mobile app, and a phone call with their new coach. Coaches monitor the participants' health data and pull out all the stops to help them follow the doctor’s orders: take their measurements, take their medications, and improve their diet and exercise routine.
